What to Bring
Definitely bring your smile, but also remember to bring with you to your child’s first appointment…

  • Dental insurance card

  • A parent or legal guardian is required to be present at your child’s first visit to our office. If you are a legal guardian, be sure to bring your guardianship paperwork.

  • Government identification for parent or legal guardian

  • We will provide patient forms electronically after scheduling your first appointment. Be sure to complete these prior to your child’s appointment.

Regular checkups are an important part of maintaining a healthy smile! The American Dental Association recommends that children visit the dentist every six months for regular checkups and routine teeth cleanings to keep teeth and gums healthy.

We understand that your family has a busy schedule, so when you’re preparing for a dental visit, please keep in mind that your child’s six-month checkup may last between 30 and 45 minutes.

Appointment Policy
A specific timeframe is allotted for your child according to his or her treatment needs. As a courtesy, Hamilton Pediatric Dentistry will make every attempt through texts, emails, and phone calls to remind you of upcoming visits. Please remember that you, as the responsible party, are ultimately responsible for your child’s appointment and as such we respectfully ask that you take a moment to record appointments on your calendar.

We require a notice of 2 Business Days to cancel or reschedule an appointment. It is our policy to allow only two “broken” appointments per family, after which the family will be dismissed from our practice.

What is a Broken Appointment?

An appointment is considered broken if any of these things happen:

  • The patient does not come for their scheduled appointment.

  • The patient is more than 10 minutes late for their appointment.

  • The patient cancels without giving reasonable notice. We require a notice of 2 Business Days to reschedule.

DENTAL INSURANCE

As a courtesy to you we will submit to all dental insurances for your child’s dental care. Please be sure to check with your carrier to determine if you are able to go to any provider. Some insurances require you to see an “in network” dentist.

Insurance policies generally cover only a portion of the total treatment cost. We ask that you pay your portion of the bill at the time of treatment. It is your responsibility to pay any balance not paid by your insurance company.

Blue Cross Blue Shield (BCBS) patients are required to pay in full for services at the time of the appointment, as a courtesy, we will submit your claim, and you will be reimbursed by BCBS directly.

We accept Michigan Healthy Kids insurance (Delta Dental ONLY) when you are referred by your general dentist for treatment.

Please bring the necessary dental insurance information to our office at your child’s first visit.

For your convenience, we also accept most major credit cards as well as Care Credit.
